I/We further authorize Alliance West Mortgage, Inc. and any investor to whom Alliance West Mortgage, Inc. may sell my mortgage to order a consumer credit report and verify other credit information, including past and present mortgages, landlord references, and release or disclose personal health information.

Alliance West Mortgage, Inc. may also utilize the services of ADVANTAGE CREDIT OR OTHER CREDIT BUREAU VENDORS to further verify my personal credit information and the information Alliance West Mortgage, Inc. obtains is only to be used in the processing of my application for a mortgage loan.
